 輕熟女反覆胃痛解黑便 確診胃癌半胃切除


台中30歲李小姐反覆胃痛解黑便，服用胃藥緩解症狀，直到日前赴醫安排胃鏡檢查，發現有2公分胃潰瘍，切片後證實為分化不良胃癌，因李小姐擔心手術留下疤痕，一度猶豫是否接受開刀治療，跟醫師討論後，決定透過腹腔鏡進行半胃切除加淋巴廓清，術後外觀只留下三個小傷口，再透過輔助性化療，李小姐恢復得相當好。（張文祿報導）亞洲大學附屬醫院肝膽胃腸科主治醫師許樹湖表示，患者沒有家族史，也沒有感染幽門螺旋桿菌，但經常胃痛、解黑便，切片結果證實為分化不良胃癌，術前影像評估已有局部擴散的情形，許樹湖說明，胃癌位居去年國人十大癌症死因第七位，好發年齡約50歲到60歲，男性多於女性，但近年來有年輕化的趨勢，初期症狀為噁心、上腹痛、脹氣與其他胃部毛病，如胃炎、胃潰瘍等，如果出現體重減輕、嚴重貧血、解黑便等，恐為胃癌末期的警訊。許樹湖指出，胃癌的成因很複雜，常見的危險因子包括遺傳、幽門螺旋桿菌、燒烤醃漬類食物、惡性貧血、慢性萎縮性胃炎等，因早期胃癌不易診斷，胃鏡是目前診斷早期胃癌的最佳利器。透過腹腔鏡進行半胃切除加淋巴廓清，除了胃可保留外，腫瘤也能清除乾淨，不僅能降低復發率、保留胃功能，術後恢復也快。許樹湖醫師提醒，如果出現腹脹、上腹痛、輕微噁心、反胃、全身倦怠、對飲食喜好改變等，且不適感持續兩週以上，應立即赴醫檢查，千萬不可擅自服用成藥，以免耽誤病情。此外，因胃潰瘍症狀與胃癌相似，但良性胃潰瘍與胃癌在病理及治療方式上是不同的，故若胃潰瘍經藥物治療四週以上仍無改善，則可能為惡性，此時應提高警覺，接受進一步檢查與治療。
